What to Include in Your Referral

To process your referral as quickly as possible, please include the following when faxing or calling:

• Patient name, date of birth, and contact number
• Insurance information and group/member ID
• Your clinical question or reason for referral
• Relevant notes, imaging reports, or prior test results
• Current medications (especially inhalers and anticoagulants)
• Your fax number for receipt of consultation reports
